This database template allows you to create a simple Survey Form for the web. Questions can be modified by a Lotus Notes user, instead of asking IT to modify the Form design. When you need a new survey, create a new database from the template, and give your "power user" access to modify the questions.

End User, "Where's IT when you need 'em? We need 2 new survey forms on the web site, and they have different questions."
IT, "Here are 2 new Lotus Notes databases. You can enter the questions yourself. Open the database, and click on the 'database configuration' button."

End User, "We need to modify question #4 on the survey."
IT, "Hi. You can do that yourself. Click on the 'database configuration' button."

End User, "We need a new survey."
IT, "Here's a new database link. Enjoy."

• Download the template (teamques2.ntf).
• Sign the database template. Adjust the ACL as needed.
• Create a new database from the template.
• Give yourself, and the servers Manager access, and the [GlobalReader] role.
• The -Default- access should be Author with "create documents" checked.
• Users that need to view the survey results should have at least Reader access and the [GlobalReader] role.
• Users may modify the database configuration, if they have at least Editor access.

• Added more views to the database. Questions will appear in the Lotus Notes database Outline.
• Added an option to color the table rows.

• Bug fix. For questions using the format "other please provide comments below", the resulting comment was not shown on the form.

• Lotus Notes database now has a "blue washed" appearance.
• Rating Format. You can now have 1-3, 1-4, 1-5, and 1-6 choices for the Survey's rating questions.
• Custom "thank you" message. You can now display a custom message when the survey is complete, or redirect the user to another URL.
• Database Configuration form now includes an "Import Questions" button. This allows you to quickly configure a new Survey by importing the settings from another Survey database.
