This database template allows you to create a simple Survey Form for the web. Questions can be modified by a Lotus Notes user, instead of asking IT to modify the Form design. When you need a new survey, create a new database from the template, and give your "power user" access to modify the questions.



Getting Started


(***This was stated above, and it's worth repeating. You need to SIGN the database template before using it.)


Next, open the database, and click on the "Database Configuration" option. This allows you to enter the Survey Questions.



(The "web server" field is used by the action "open web survey form". It's a nice reference so user's know the URL to the survey form.)






Click on "Open Web Survey Form". This should open the web form.

Tips


You can add HTML to your questions. Simply include the HTML in the question text. Some examples:
